1. Acceptance of Terms
These Terms of Service ("Terms") are a binding agreement between you and GoodLife Health LLC and its affiliated professional medical entity (collectively, "GoodLife Health," "we," "us," or "our"). By accessing goodlifehealth.ai, signing up for membership, or using the patient portal, you agree to these Terms and our Privacy Policy.
If you do not agree, do not use our services.
2. Who can use GoodLife Health
To use GoodLife Health you must:
- Be at least 18 years old
- Be a U.S. resident located in a state where we operate
- Provide accurate and complete information during signup and intake
- Have the legal capacity to enter into a binding agreement
We may refuse, suspend, or terminate service at our discretion, including when clinical care is not appropriate for you, when accurate identification cannot be verified, or when these Terms are violated.
3. Nature of services and clinical relationship
GoodLife Health offers a Direct Primary Care telehealth membership. Your membership provides access to licensed clinicians for primary care, hormone optimization, medical weight loss, and related clinical services, depending on the membership tier and clinical recommendations.
Clinical care is provided by licensed clinicians who exercise independent professional judgment. The platform and its technology do not provide medical advice. A clinical relationship is established only when an authorized clinician accepts you as a patient and provides care.
Telehealth has limitations. GoodLife Health is not a substitute for emergency care. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. We do not provide controlled substance prescriptions in violation of state or federal law.
4. Eligibility, intake, and provider judgment
Eligibility for clinical care is determined by a licensed clinician based on your intake, history, labs, and overall clinical picture. Submitting an eligibility check or paying a membership fee does not guarantee that any specific treatment, including hormone therapy or GLP-1 medications, will be prescribed.
Your clinician may decline to prescribe any medication or recommend any treatment if doing so is not clinically appropriate. Clinical recommendations may change over time based on your labs and response to care.
5. Membership, billing, and the 3-month commitment
5.1 Pricing
Membership pricing is shown on our pricing page and may change from time to time. Current pricing applies to new members from the date of signup.
5.2 Foundation membership: 3-month upfront commitment
The Foundation tier ($179/month at the effective date of these Terms) requires a 3-month minimum commitment. At checkout, you will be charged $537 upfront, covering your first three months of membership. After your initial 3-month term, your membership continues on a month-to-month basis at $179/month (or then-current monthly rate) until you cancel.
The 3-month upfront structure reflects the clinical reality that meaningful primary care, lab evaluation, and protocol adjustment require at least three months to do properly.
5.3 Higher tier memberships
Hormone Optimization, Medical Weight Loss — Semaglutide, and Medical Weight Loss — Tirzepatide tiers are billed monthly and continue until you cancel. Upgrades from the Foundation tier occur after a clinical conversation and your authorization.
5.4 Auto-renewal
All memberships renew automatically until canceled. By signing up, you authorize us to charge your payment method on the recurring billing date.
5.5 Failed payments
If a payment fails, we may suspend your membership and clinical access until payment is resolved. We may retry payment and notify you. We may terminate membership for repeated payment failures.
6. 30-day money-back guarantee and cancellation
6.1 30-day money-back guarantee
If within the first 30 days of your initial Foundation membership you decide GoodLife Health is not right for you, you may request a full refund of your initial membership payment. To request the refund, contact us at info@goodlifehealth.ai within 30 days of your initial charge. The refund covers membership fees only. We cannot refund the cost of medications dispensed or laboratory services rendered.
6.2 Cancellation after the first 30 days
You may cancel your membership any time. Cancellation stops future billing. We do not pro-rate refunds for partial months, and the 3-month upfront payment for the Foundation tier is not refundable after the 30-day money-back window (except as required by law).
6.3 Termination by us
We may terminate or suspend your membership if you violate these Terms, behave abusively toward staff, provide false information, or for clinical reasons (for example, if telehealth is no longer appropriate for your care). In the event of termination by us without cause, we will refund unused membership fees on a pro-rated basis.
7. Medications and pharmacy
When clinically indicated, your clinician may prescribe medications including bioidentical hormone therapy, branded GLP-1 medications, and other treatments. Prescriptions are filled by independent, licensed pharmacies. Branded GLP-1 medications (for example, Wegovy or Zepbound) are obtained directly from the manufacturer or through your insurance. GoodLife Health does not provide compounded GLP-1 medications.
All medications carry potential benefits and risks. Your clinician will discuss the benefits, risks, and alternatives of any prescribed medication with you, and you acknowledge these when accepting a prescription.
Medication costs may be included in your membership tier or billed separately depending on the medication and your plan. Pricing is disclosed before any medication is shipped.
8. Patient responsibilities
You agree to:
- Provide accurate, complete, and current information
- Update your information when it changes
- Follow your clinician's reasonable instructions
- Disclose other medications, supplements, conditions, and providers
- Use medications only as prescribed
- Not share your account or medications with others
- Communicate with the clinical team about side effects or concerns
9. Communications and consent to contact
By signing up, you agree that we may contact you by email, SMS, push notification, and phone for clinical care, account matters, and (if you opt in) marketing communications. Standard message and data rates apply for SMS. You may opt out of marketing at any time. You cannot opt out of clinical and transactional communications while you remain a member.
10. Research participation
To advance clinical understanding of hormone optimization, metabolic care, and related therapies, GoodLife Health maintains a de-identified research dataset. This dataset is used internally to improve our clinical protocols and may be used for research and product development purposes.
During signup, you will be presented with a separate research consent option. Research participation is voluntary and is not a condition of membership. You may opt out at any time through your patient portal or by contacting us. Information used for research is de-identified and stripped of HIPAA identifiers before use.
11. Intellectual property
All content on goodlifehealth.ai (including text, graphics, logos, photographs, clinical protocols, and software) is owned by GoodLife Health or its licensors and is protected by copyright, trademark, and other laws. You may use the site only for personal, non-commercial purposes. You may not copy, modify, distribute, or create derivative works without our written permission.
12. Disclaimers and limitation of liability
Services are provided "as is" and "as available." To the maximum extent permitted by law, we disclaim all warranties, express or implied, including warranties of merchantability, fitness for a particular purpose, and non-infringement.
To the maximum extent permitted by law, GoodLife Health, its officers, employees, contractors, and affiliates will not be liable for any indirect, incidental, consequential, special, punitive, or exemplary damages arising from or related to your use of the services. Our total liability for any claim related to the services is limited to the amount you paid us in the twelve months preceding the claim.
Nothing in this section limits liability for fraud, willful misconduct, or any liability that cannot be limited or excluded under applicable law.
13. Indemnification
You agree to indemnify and hold harmless GoodLife Health, its officers, employees, contractors, and affiliates from any third-party claim, loss, liability, or expense (including reasonable attorneys' fees) arising from your violation of these Terms, your misuse of medications, or your provision of false or misleading information.
14. Arbitration and class action waiver
Please read this section carefully. It affects your legal rights.
You and GoodLife Health agree that any dispute arising out of or relating to these Terms or our services will be resolved by binding arbitration administered by the American Arbitration Association under its Consumer Arbitration Rules, rather than in court, except that you may bring a claim in small claims court if the claim qualifies.
You and GoodLife Health agree that disputes will be brought only in an individual capacity, and not as a plaintiff or class member in any class, collective, or representative proceeding. The arbitrator may not consolidate claims or preside over any form of representative or class proceeding.
You may opt out of this arbitration provision within 30 days of first agreeing to these Terms by sending written notice to info@goodlifehealth.aiwith the subject "Arbitration Opt-Out." Opting out does not affect any other part of these Terms.
This arbitration provision does not apply to claims for injunctive relief related to intellectual property or unauthorized account access.
15. Governing law and venue
These Terms are governed by the laws of the State of Arizona, without regard to its conflict-of-laws principles. To the extent a claim is not subject to arbitration, the exclusive venue is the state and federal courts located in Maricopa County, Arizona, and you consent to personal jurisdiction in those courts.
16. Changes to these Terms
We may update these Terms from time to time. Material changes will be communicated through the patient portal or by email at least 30 days before they take effect. Continued use after the effective date of changes constitutes acceptance.
