How long can you safely stay on a GLP-1 medication? There is no fixed time limit in the medical literature. The SELECT trial followed more than 17,000 people on semaglutide 2.4 mg for roughly four years with a safety profile consistent with the shorter trials. SURMOUNT-1 and SURMOUNT-4 followed tirzepatide patients beyond two years. In practice the right duration is not a number of months but a clinical condition, which is that the lowest effective dose keeps working, the side effects stay manageable and the quarterly panel keeps confirming that the therapy is doing more good than harm.
- The SELECT trial followed over 17,000 people on semaglutide for about four years without new safety signals.
- Weight regain after stopping is well documented, so duration is a clinical decision rather than a calendar rule.
- The right dose is the lowest one that holds the result, not the highest one tolerated.
- Our $299 flat fee never rises with the dose and the pharmacy bills the medicine separately at cost.
- Labs every 90 days decide continuation, dose and any taper, never a subscription deadline.
What the long term trials actually show
The concern behind this question is usually whether a years-long medication quietly accumulates risk. The long record so far says the opposite. SELECT was designed to answer a different question, whether semaglutide reduced cardiovascular events in people with obesity and established cardiovascular disease. It ran long enough to serve as the best duration evidence we have. Over a median of about four years the treated group had fewer major cardiovascular events than placebo and no emergent safety problem attributable to duration.
Tirzepatide has a shorter record but the same shape. SURMOUNT-1 ran 72 weeks and its extension work continued beyond that and SURMOUNT-4 showed that patients who stayed on the medication kept the weight off while those who stopped regained most of it within about a year. Duration and maintenance are the same question. The trials answer it together.
Why stopping is usually the risky move, not staying
The data on discontinuation is the part of this conversation most programs skip. In SURMOUNT-4, participants who switched from tirzepatide to placebo after 36 weeks regained roughly two thirds of their lost weight over the following year. STEP 1 extension showed the same pattern for semaglutide. The biology is not mysterious. The medication modulates appetite and metabolic signaling and when it leaves the system the signaling reverts toward its old set point.
That finding cuts two ways and we say both. Staying on a GLP-1 indefinitely is a legitimate clinical strategy when the panel and the symptoms support it. It is also legitimate to step down to the lowest effective dose or to transition to maintenance without medication, when a patient has built the nutrition, activity and behavioral structure that lets the result hold. What is not legitimate is a program whose business model requires the prescription never to change, because that program has no reason to ever ask whether the dose can come down.
How we decide duration with a patient
At GoodLife Health the duration conversation happens at the Biomarker Audit every 90 days and it reads four signals together.
- The trend in the panel. ApoB, HbA1c, fasting insulin, hs-CRP and the rest of the audit tell us whether the metabolic trajectory is still improving, flat or regressing.
- Symptoms and tolerability. Nausea, reflux, fatigue and the quieter signals that never make the intake form get asked about directly at each audit.
- The dose floor. We look for the lowest dose that holds the result and we test that floor deliberately rather than assuming the current dose is still needed.
- Life context. New medications, pregnancy plans, surgery, insurance transitions and travel all legitimately change the calculus and belong in the chart.
If all four point to continued benefit, staying on therapy is the evidence based answer and we say so plainly. If the panel shows the goal state is being maintained on a reduced dose, we reduce it, because our guidance has no financial motive to keep the dose high.

The fee structure answers the question underneath
The question underneath is usually financial, whether staying on a GLP-1 for years means staying in a program that charges by the dose forever. Here the structure matters more than any reassurance. The membership is one flat $299 per month covering the clinician relationship and four Biomarker Audits a year. It is unchanged whether you take the lowest effective dose or the highest clinically required one. There is a two month commitment to start, a single $598 charge covering the first two months and from month three you can cancel anytime.
The medication is the second charge and it is billed by the pharmacy directly at pharmacy cost, with no markup, no molecule upcharge and no dose escalation fee on our side. That separation is deliberate. If the fee never rises with the dose and nobody in the model earns on the prescription, then the recommendation to stay, step down or stop can be made on the medicine alone.
Situations that genuinely end GLP-1 therapy
Honesty requires naming the cases where we stop rather than continue. A planned pregnancy changes the conversation immediately and GLP-1 therapy is typically stopped ahead of conception. A history of medullary thyroid carcinoma or MEN2 in the patient or family is a contraindication we screen for at intake. Pancreatitis in the history prompts a specialist level review before any continuation. And a patient who has held their goal weight, rebuilt their metabolic markers and wants to test life without the medication can do that in a structured taper with a recheck scheduled, which is a far safer experiment than an abrupt stop.
Frequently Asked Questions
Is it safe to stay on a GLP-1 for years?
The best available evidence supports it. The SELECT trial followed more than 17,000 people on semaglutide 2.4 mg for a median of roughly four years and found fewer cardiovascular events in the treated group with no new duration related safety signal. Continuation still deserves periodic review against labs and symptoms.
What happens when you stop a GLP-1?
Most people regain a substantial share of the lost weight within a year. In SURMOUNT-4, participants who stopped tirzepatide regained about two thirds of their lost weight over twelve months. A structured taper and a maintenance plan reduce that risk and an abrupt stop maximizes it.
Do you have to stay on a GLP-1 forever to keep the weight off?
No. Some patients maintain on the lowest effective dose and some transition off successfully with nutrition, activity and behavioral structure in place. The right answer for a given patient is read off the quarterly panel, not assumed in advance.
How long does it take to know if the dose can come down?
One ninety day audit cycle. We reduce the dose deliberately, then recheck weight, symptoms and the metabolic panel at the next Biomarker Audit before deciding whether the reduction holds.
Are there medical reasons you cannot stay on a GLP-1?
Yes. A personal or family history of medullary thyroid carcinoma or MEN2, a history of pancreatitis requiring specialist review and pregnancy planning are the most common. We screen for all of them at intake and revisit them at every audit.
Does GoodLife Health charge more for a higher dose?
No. The membership is a flat $299 per month regardless of dose and the medication is billed by the pharmacy at pharmacy cost with no markup. Nobody in our model earns more when the dose goes up, which is why the dose can follow the medicine rather than the business model.
What is the two month commitment at sign up?
The membership starts with one $598 charge that covers the first two months together, including the first Biomarker Audit and the clinical intake. From month three it is $299 monthly and you can cancel anytime.
One last thing
Duration questions are usually trust questions in disguise and the honest resolution is structural. Ask any program who earns more when your dose climbs. If the answer is the program itself, the duration conversation you are having is not about medicine. In our model the fee is the same at every dose, so the answer to how long is decided by the panel, the symptoms and you.
