Seeing a doctor should not cost you a day.
You call the clinic and wait on hold. The first appointment is weeks away. On the day, you drive across town, find parking, and sit in a waiting room for longer than you will spend with the doctor.
Then you get eight minutes, a basic panel, and the sentence you have heard more than once: “your labs are normal.” Meanwhile the weight will not move, the energy is gone by mid-afternoon, sleep breaks at 3 a.m., and your focus or mood is not what it was.
Maybe you have tried something since: a GLP-1 that worked and then stalled, a hormone conversation that went nowhere, an app that prescribed in a day and never looked at you, or a concierge quote you could afford but did not trust.
What most Americans die of is what a basic visit does not look for.
Heart disease is the leading cause of death in the United States, and six in ten American adults live with at least one chronic disease, according to the CDC. More than eight in ten adults with prediabetes do not know they have it.
Your weight, blood sugar, cholesterol, blood pressure, hormones, thyroid and inflammation work as one system, and they can drift for years before anything shows up as a diagnosis. A basic panel looks at a few of them, once a year, against a population range.
Fixing the rushed visit is not the same as measuring the right things. Plenty of practices, insurance-based or direct, still run the same basic panel and wait for something to go wrong. Precision care measures what predicts the next twenty years, like ApoB, Lp(a), fasting insulin and hs-CRP, and follows it every quarter.
Precision telehealth primary care, word by word.
Every word in what we do means something specific.
- Telehealth
- Video visits and messaging with your care team from home, anywhere in the 50 states. The only trip is a walk-in blood draw at a Quest location near you.
- Data-driven
- We start with a baseline cardiometabolic panel at Quest Diagnostics, then draw again every 90 days. Every result is yours, in your portal.
- Precision
- Your plan is built on your own numbers, read against your age, history, medications, risk, symptoms and last result.
- Direct primary care
- You pay a membership, not a visit. Your clinician’s time goes to you, not to insurance billing.
- Longevity
- Measuring what predicts the next twenty years, like ApoB and Lp(a), before it becomes a diagnosis.
- Chronic disease management
- Prediabetes, type 2 diabetes, cholesterol, blood pressure and fatty liver, followed every quarter instead of once a year.
Three areas of care, one clinician holding the whole picture.
Cardiometabolic optimization
- Type 2 diabetes and prediabetes
- Insulin resistance and a rising A1c
- High ApoB, LDL or triglycerides
- High blood pressure
- Labs your doctor called normal
Medical weight loss
- Weight that comes back after every diet
- A plateau on a GLP-1 you already take
- A dose that keeps climbing
- Side effects nobody is managing
Hormone optimization
- Perimenopause and menopause
- Low testosterone
- A thyroid that is “in range” but not right
- Symptoms a doctor dismissed

$598 today. $299 a month from month three.
$598 covers your first two months. From month three, $299 a month, month to month. Year one is $3,588 in all.
We sit between the $20,000-a-year concierge practice and the $99-a-month app. We charge for the clinician and the data, never for the drug or the dose, so nothing in our fee depends on what you are prescribed.
The membership works alongside your insurance; keep it for the hospital and the emergency. What it replaces is the eight-minute primary care visit. The fee is generally eligible for HSA or FSA dollars.
If your clinician cannot accept you, your $598 comes back.
If your clinician cannot accept you into care, your $598 is refunded in full. Once you are accepted, the first two months are non-refundable. Medication billed by a pharmacy follows that pharmacy’s own refund policy.

A trend only exists if someone measures it.
An A1c of 5.3 one year, 5.5 the next and 5.7 now is a story. Each number on its own looks unremarkable; together they say something has changed, years before it has a name.
Every quarter that goes unmeasured is a quarter of change nobody saw. Checking takes five minutes, from wherever you are reading this.
Who this is for, and who it is not.
You want the full picture measured, a clinician who stays, and a plan built on your own numbers, and $299 a month is a choice, not a strain.
You want the cheapest prescription, a prescription without labs, compounded medication, peptides or pellets; you expect your insurance to be billed or a prior authorization run for you; you are under 18, pregnant or planning pregnancy on a GLP-1; or you need urgent, pediatric or full-scope primary care. We treat patients in the United States only.
Questions, answered.
Do you take insurance?
No. You pay the membership directly, which is what buys your clinician’s time. Keep your insurance for hospital and emergency care. The fee is generally eligible for HSA or FSA dollars.
What does medication cost?
Whatever your pharmacy charges. Medication is prescribed only when clinically indicated, to the pharmacy you choose. We take nothing on it, and our fee is the same whatever is prescribed.
What if I am not accepted?
Your $598 is refunded in full. Once you are accepted, the first two months are non-refundable.
Who are the clinicians?
Board-certified physicians licensed in all 50 states, working to clinical protocols designed by our co-founder, Kristin Makinajyan, DNP, FNP-BC.
Will I be prescribed a GLP-1 or hormone therapy?
Only if your clinician decides it is indicated. We test first and treat second, and the membership never promises a medication.
Do I have to go anywhere?
Only to a Quest location near you for your blood draws. Everything else happens in your portal and by video.
Is a video visit as good as sitting in an office?
For this kind of care, the visit is a conversation about your history, your symptoms and your results, and that works by video. The physical part, your blood draw, happens in person at Quest. If you ever need an exam, urgent care or a specialist, your clinician tells you where to go.
Your next appointment can be at your kitchen table.
You have been told you are fine by people who did not measure enough to know. Start with the full picture, a board-certified physician you can see from home, and a plan built on your own numbers.
If your clinician cannot accept you, your $598 is refunded in full.
Or close this tab, and keep the eight-minute visit.
